If you’re under the care of a medical professional for certain problems, I’ll need them to know that I’m helping you, and for them to say it’s okay.

All you need to do is download this note, and have your doctor or other medical professional to sign it.

Then you sign it too - and send it back to me as a photograph or a scan via e-mail.
